MetLife High and MetLife Basic
Both provide a PPO network, meaning you can go to any provider you choose, but it will cost less if you stay in-network.
Aetna Dental Maintenance Organization (DMO)
Only available in certain geographic areas and if eligible, will be displayed during online enrollment. You must designate a Primary Care Dentist (PCD) for you and your covered dependents when you enroll.
In Hawaii and Guam, your dental option is available through Hawaii Dental Service. In Puerto Rico, your dental option is available through Triple-S.
Vision

• Annual in-network vision refraction and routine eye examination covered at 100%
• Lenses with copays or a $160 contact lens allowance once every 12 months
• A $160 frame allowance once every 12 months

• A 10% discount when you use your Macy’s or Bloomingdale’s credit card at
