Live Healthy incentives generally are paid as one-time contributions to your HSA or HRA, but certain collectively bargained colleagues and other special groups may receive payroll premium deductions instead.
The company will provide HSA/HRA seed contributions and/or Live Healthy program incentives only to colleagues who are actively at work and enrolled in a Cigna medical option at time of funding; colleagues on leave of absence will not receive any HSA/HRA funding or payroll premium reductions unless and until they return to work. COBRA participants may participate in Live Healthy programs but are not eligible to receive any rewards/incentives. Payout form and schedules may vary.
Live Healthy program participation and results are con dential and protected by federal and state privacy laws, and individual participant results are not shared with the company.
The company match percentages re ect the Macy’s, Inc. national 401(k) Plan. Colleagues covered by a collective bargaining agreement may receive a lower match rate than the amounts indicated. Must be 21 years old. Automatic enrollment takes place after 1,000 hours of service in the rst year. Some colleagues may be eligible for immediate participation.
